Napoli. «We are starting to have a clear picture of the damage caused by the pandemic and we are recording a real emergency with regard to post-Covid dilated heart disease, which in the cardiology field we could define as the disease of the future». The alarm was raised by Ciro Mauro, director of the complex structure of Cardiology with Utic of the Cardarelli, in his capacity as president of the Congress “Hot topic in cardiology”, which for two days brought the major experts from all over Italy to Naples.

Also present were the general director of the Cardarelli of Naples Giuseppe Longo, the health director Giuseppe Russo and the president of the Order of Physicians Bruno Zuccarelli. And one of the most worrying realities is precisely that which emerges from a privileged observatory, the Cardarelli of Naples, and confirmed by the specialists who deal with emergency-urgency at a national level.

Mauro explains that "post-ischemic and post-Covid dilated cardiopathy will be the disease we will have to deal with in the coming months and years. There is a close link with the SarsCoV2 virus, because the heart and lungs are interconnected, the suffering of one is reflected in the other".

Fortunately, the progress made at the pharmacological level now allows us to treat this disease more effectively and Cardarelli now guarantees cutting-edge treatments and procedures. «We were the first – Mauro recalls – to formulate a percutaneous treatment protocol for aortic stenosis in collaboration with Federico II, and in recent days we have also started using the coronary laser for cases refractory to conventional therapy.

Today, the Cardarelli is among the reference structures at a national level for the treatment of acute coronary syndrome.
with approximately 450 treatments per year and specific pathways for atrial fibrillation, chest pain and a specific PDTA of the internal stroke network. The only hospital company in Campania identified as a Covid Hemodynamics Center, with a pathway that allows treating patients with acute coronary syndrome despite being affected by Covid". Another emergency, specialists reveal, is given by post-Covid myocarditis, increasingly frequent.

"Everyone now has the perception that Covid only attacks the lungs, but this is not the case. The virus affects our body systemically," explains Mauro, "it is no coincidence that we are seeing a high number of cases of myocarditis, even serious ones, which in the long run, especially if left untreated, can be fatal."

In other words, another face of what is increasingly being called “long Covid” emerges from the Naples congress. Central to the treatment of these and other cardiac pathologies is the role of facilities such as the Cardarelli in Naples, a reference point in the emergency-urgency network for all of Southern Italy. And this is precisely why Naples was chosen for this two-day update.

"Cardiovascular diseases are a constant challenge," continues the head physician of Cardarelli. "The management of patients at high cardiovascular risk today represents an integrated work model between specialists, where the exchange of information and the coordination of the various figures involved is fundamental to optimize therapeutic results and use available resources rationally."

The event in Naples was attended by cardiologists, internists, neurologists and general practitioners. One of the points on which everyone agreed was that chronic ischemic heart disease, atrial fibrillation, hypertensive heart disease, acute coronary syndrome with pharmacological management, need to be addressed today with method and with constant discussion also at an interdepartmental level.

"The comparison and continuous updating - concludes Mauro - are essential. Thanks to days like these we are able to give the specialist and the general practitioner a broad overview of recent acquisitions, innovations and consolidated therapies with a view to a better review of the data flow of management of the patient at high cardiovascular risk".
