AVELLINO – He arrived at the emergency room in desperate condition, suffering from one of the most feared vascular emergencies. A 71-year-old man, resident in the province of Naples, underwent emergency surgery at the San Giuseppe Moscati Hospital in Avellino for a serious thoracoabdominal aortic dissection, a very high-risk condition requiring immediate intervention and highly specialized expertise.
The dissection had caused a tear in the inner wall of the aorta, creating two distinct blood flow channels and compromising the blood supply to vital organs. This is a condition that cannot be delayed and, without prompt treatment, can be fatal.
The team from the Vascular Surgery Unit, led by Loris Flora, performed a complex procedure in the hospital's new hybrid operating room, a high-tech space combining traditional surgery and advanced diagnostic imaging. Surgeons operated through a femoral access and, using ultrasound guidance, performed an aortic septotomy, a controlled incision of the internal lamella that separated the so-called "false lumen" from the "true lumen" created by the dissection. "This phase is particularly delicate, requiring experience and precise maneuvering to guide the cable across the lamella and intervene with the electrosurgical unit at the right point," Flora explained.
After restoring a single flow channel, a thoracoabdominal endoprosthesis was implanted, a device that stabilizes the aorta, restores proper blood circulation, and protects the arteries leading to the abdominal organs. The surgery was successful, allowing two highly complex procedures to be combined in a single session.
Post-operative management, entrusted to the cardiac anesthesiologists led by Arianna Pagano, was also crucial. Today, the patient is in good clinical condition and is nearing discharge from the Vascular Surgery department, following an operation that represents an example of highly specialized healthcare in the region.
