Salerno – Six days on a stretcher in the emergency room and, upon transfer to the ward, the offer of admission to the men's ward, in a room shared with five men.
This is the reconstruction provided by Antinoo Arcigay Napoli and the Trans Napoli Association on the case involving Iolanda, a transgender woman, who allegedly refused the arrangement, deeming it incompatible with her gender identity and "undermining her dignity."
The matter, which sparked reactions and calls for institutional intervention, was resolved—according to associations—with mediation by the Campania Region and the involvement of the health department of the San Giovanni di Dio e Ruggi d'Aragona hospital in Salerno, which identified a more suitable location.
Hospitalization in the emergency room and the transfer proposal
According to Arcigay Napoli, Iolanda remained on a stretcher in the emergency room for six days, waiting for a bed. Upon admission, she was offered a transfer to a men's ward, sharing a room with other male patients (five, according to the reconstruction).
According to the association, the reason given by the facility was based on the fact that the patient, "biologically," was considered a man and therefore ineligible for admission to the women's wards. This is, in this instance, the explanation provided by the associations and attributed to the facility.
"Psychological violence": refusal and leaving the hospital
Faced with that solution, Iolanda – the promoters of the report report – decided to refuse hospitalization and leave the hospital despite her health conditions described as “precarious”.
The woman reportedly experienced the proposal as "real psychological violence," choosing to expose herself to risks to her physical safety rather than accept an arrangement perceived as humiliating.
According to the associations, the story also revolves around respect for the individual and access to care: leaving the hospital is described as a "forced" act due to the lack of dignified alternatives.
Reporting: from the support network to the Regional Observatory
After the incident, Iolanda reportedly contacted Loredana Rossi of the Naples Trans Association. The support network reportedly involved Antonello Sannino, president of the Campania Region's LGBTQIA+ Observatory, with the aim of obtaining a swift resolution and bringing the case to the attention of regional institutions.
Arcigay Napoli claims that, once they arrived at the hospital, the representatives encountered "a wall of intransigence." In their complaint, the emergency room manager allegedly denied the patient "dignity and respect."
The associations speak of a "clear violation" of ethical principles and constitutional protections, going so far as to describe the incident as one in which "a woman's health" risked being sacrificed "due to prejudice and bureaucracy." These, too, are assessments and accusations made by the individuals who released the statement.
The mediation of the Region and the turning point in the evening
The breakthrough, according to Arcigay Napoli and the Trans Napoli Association, came late in the evening thanks to direct intervention by the Campania Region. President Roberto Fico, with the support of councilors Pecoraro and Morniroli, was personally following the matter.
According to the reconstruction, the mediation led to an "awareness raising" on the part of hospital management and the involvement of the Ruggi health management, which identified a placement deemed "dignified and appropriate" for Iolanda's gender identity. In one of the versions circulated, the associations also report an apology to the patient for what happened.
The crux: rules, departments and protection of dignity
The case reopens an issue that, in healthcare facilities, can lead to conflicts between organizational practices, ward placement, and gender recognition, especially when shared rooms and standardized care pathways are involved. This is where associations are calling for clearer protocols: how to manage admissions, privacy, and safety, preventing patients from having to choose between care and dignity.
Arcigay Napoli emphasizes that the incident represents "a point of no return" and states that President Fico has expressed his willingness to take immediate action to draft specific guidelines to equip healthcare facilities with adequate protocols. The stated goal is to prevent similar incidents from recurring and to ensure "absolute priority" for the right to health, while respecting the dignity of all, "without distinction."
The request: uniform protocols to avoid new cases
The focus, therefore, is not only on the outcome of a single episode, but also on the method: consistent procedures across hospitals, clear references for emergency rooms and departments, and tools that help healthcare personnel manage complex situations without placing the burden on the patient.
For associations, managing hospitalization cannot be reduced to a binary and bureaucratic choice, especially when the concrete consequence is a person forgoing treatment. Hence the call for rules and operational guidelines that balance safety, privacy, clinical appropriateness, and respect for gender identity.
