Stomach cramps and diarrhea appearing together are among the most unpleasant and sudden digestive symptoms. In many cases, these are self-limiting episodes that resolve within a few hours, but in others, their simultaneous appearance can signal situations requiring attention. Understanding the causes and knowing how to treat them is essential to managing them properly.

Why do stomach cramps and diarrhea occur together?

The combination of abdominal cramps and diarrhea is often the result of accelerated intestinal motility: the intestine contracts more rapidly than normal, causing both cramping pain and accelerated stool passage. The most common causes of this mechanism include:

  • Viral or bacterial gastroenteritis: This is the most common cause of cramps and acute diarrhea. Viruses like norovirus or bacteria like Salmonella cause inflammation of the intestinal mucosa, which accelerates transit and causes pain.
  • Food poisoning: contamination of food with bacterial toxins often causes sudden and intense symptoms, with cramps and diarrhea in the hours following the offending meal.
  • Irritable bowel syndrome (IBS): in the diarrhea-predominant form (IBS-D), the stomach cramps and diarrhea are the key symptoms, often triggered by stress or specific foods.
  • Food intolerance: lactose or gluten (in sensitive individuals) can cause cramps and diarrhea in the hours following ingestion.
  • Anxiety and acute stress: the autonomic nervous system, in response to a situation of strong tension, can drastically accelerate intestinal transit.
  • Use of antibiotics: microbiota disruption caused by antibiotics can manifest itself with cramps and diarrhea for the entire duration of therapy.

Remedies to manage cramps and diarrhea

In most cases, acute episodes of stomach cramps and diarrhea are managed with some practical measures:

  • Abundant hydrationDiarrhea causes significant fluid and electrolyte loss. Drinking water, clear broth, or oral rehydration solutions is the top priority.
  • Light and progressive nutrition: in the initial stages, it is best to limit yourself to easily digestible foods such as white rice, cooked carrots, bananas and rusks, gradually reintroducing other foods.
  • Bowel rest: avoid irritating foods such as spices, fats, alcohol and coffee during the acute phase.
  • Antispasmodics for cramping pain: When abdominal cramps are severe and interfere with daily life, an antispasmodic can offer quick relief. Buscopan is an example of a drug against stomach cramps and diarrhea which works by relaxing the smooth intestinal muscles, reducing painful spasms.

When Stomach Cramps and Diarrhea Require Medical Attention

Not all episodes resolve spontaneously. It's therefore important to consult a doctor if you develop additional symptoms, such as blood or mucus in your stool or a high fever (over 38,5°C).

Stomach cramps and diarrhea that occur together are almost always manageable with rest, hydration, and, in cases of acute pain, targeted antispasmodic support. The key is to distinguish a self-limiting episode from one that requires medical evaluation, paying attention to the warning signs described above.
